Louisa DaCosta Trust recognizes the importance of privacy and community concerns about the collection, use, disclosure and security of personal sensitive and health information which Louisa DaCosta Trust may collect.

Louisa DaCosta Trust may collect personal sensitive and health information about patients of SA Public Hospitals and their health professionals.

We will only collect this information as it is necessary for us to provide the financial assistance or help that has been requested or provided. The kind of information we collect and our use of that information will depend upon the purpose for which it was collected but may include information when:

  • A health professional registers with Louisa DaCosta Trust to enable requests for financial assistance to be submitted on behalf of their patients.
  • When a request for financial assistance is submitted for consideration by Louisa DaCosta Trust.

There is any communication that may occur between Louisa DaCosta Trust, the health professional and the patient or any other friend or family member of that patient.
We respect the information that is shared with Louisa DaCosta Trust. It is the policy of Louisa DaCosta Trust not to sell trade or distribute any personal information to any other party without that person’s written permission.

Requests for financial assistance

Requests for financial assistance are forward to us by health professionals and can contain personal sensitive and health information about a patient and their carers and family. We receive this information from your health professional on the understanding that the health professional and/or their organisation has:

  • Complied with all applicable State and Federal Privacy legislation
  • Obtained permission for the information contained in the request for financial assistance to be provided to 3rd parties including Louisa DaCosta Trust and that this permission has not been revoked.

Feedback

Louisa DaCosta Trust may receive feedback from a patient or a friend or family member of a patient about the financial assistance that may or may not have been provided by Louisa DaCosta Trust.

Louisa DaCosta Trust respects any information that is provided to it and it will not be disclosed to any other party without that person’s written permission.

Internet Policy

We do not collect any personal information on visitors to our website. We do not use “cookies” or other technology to track visits and may only log the following for statistical purposes:

  • The number of page hits;
  • The number of unique sessions as identified by server address and top-level domain name.
  • The pages accessed or downloaded by session;
  • The type of browser being used.

Collection, Use, disclosure of Information

Louisa DaCosta Trust respects the information that is provided when health professionals submit requests for financial assistance. This information is only used by Louisa DaCosta Trust to assess and consider the request for financial assistance, except for the need to forward information to the suppliers of goods or services as set out in the request.

In addition we may from time to time we may contract out some of our functions (for example – mailing) to external contractors or service providers and as a consequence of that we may disclose personal information to these external contractors or service providers but only to the extent that the services requested can be carried out.

The books and records of Louisa DaCosta Trust are audited by an independent firm of auditors. In the normal course of their audit, the auditor’s and their staff may access personal information maintained by Louisa DaCosta Trust.

Louisa DaCosta Trust will endeavour to take all reasonable steps to keep secure information that we hold about you. Your information is stored on secure servers.

In addition our employees and contractors who provide services related to our information systems are obliged to respect the confidentiality of any personal information held by Louisa DaCosta Trust. However, Louisa DaCosta Trust will not be held responsible for events arising from unauthorised access to the personal information that is contained in its books and records.

Access to information

We will provide reasonable access to the information we hold about as quickly as possible in accordance with privacy principles.

If we refuse to give you access to information we will give a reason for refusal. Generally if requested we will amend any personal information which you demonstrate is inaccurate, incomplete or not current, and will remove any information that is not relevant. If we disagree with your view on these matters we will keep a note on the file setting out your view of the information held.

Disposal of information

We will not keep any information, which is no longer relevant, or any information, which we are no longer, required to retain. When information is no longer required it will be securely disposed of.

Changes to Privacy Policy

We reserve the right as it may be necessary to review or revise or make changes to our Privacy policy and will notify you of those changes by posting those changes on our web site.
